• Monitor and respond to scheduling changes, including evenings and weekends as needed.
• Coordinate with clients, families, and Care Professionals to resolve issues before they escalate.
• Maintain accurate records in scheduling and care management systems.
• Identify opportunities to expand and improve care for existing clients.
• Support Care Professional engagement and contribute to a positive team culture.
