Job summary

ArchWell Health in Tucson, AZ, is seeking a Referral Coordinator to guide our senior members through the healthcare system. You will partner with a multidisciplinary team to identify care needs, coordinate referrals to external providers, and ensure timely access to the right care settings for each member.

You will document referrals, support families, and uphold HIPAA standards while helping members overcome barriers such as transportation or language needs.

Qualifications

• 2 years of experience in referral management or care coordination.
• H.S. Diploma or equivalent required.
• Experience with EMR systems, ideally eClinicalWorks or similar.
• Fluency in Spanish or languages spoken in service areas.
• Basic medical terminology and CPT/ICD-10 knowledge.

Responsibilities

• Coordinate referrals to external providers to meet care needs.
• Document referrals to maintain accurate records.
• Partner with care team to understand member care needs.
• Promote clear communication among care team.
• Assist with barriers (transportation, interpreters, prescriptions).
• Be system navigator for members and families.
