Job summary

ArchWell Health seeks a Referral Coordinator to guide members through the healthcare system, coordinating internal and external referrals and ensuring timely access to specialists. You will maintain accurate documentation and work closely with a multidisciplinary team to remove barriers to care.

The role requires 2+ years in referral management or care coordination, experience with EMR systems (preferably eClinicalWorks), and fluency in Spanish or other community languages.

Qualifications

• Must have 2 years of experience in referral management or care coordination.
• Experience with EMR systems, ideally eClinicalWorks or similar.
• Fluency in Spanish or languages spoken by the communities served.

Responsibilities

• Coordinate referrals to external providers, vendors, and services to ensure care needs are met.
• Document member referrals to ensure records are accurate and current.
• Partner with care team to understand member care needs and gaps.
• Promote clear communication among care team and families.
• Be the system navigator and point of contact for members and families.
