ArchWell Health is seeking a Referral Coordinator in Denver, CO to guide members and families through the care system, ensuring timely access to the right specialists and services. You will collaborate with a multidisciplinary team to identify gaps in care and coordinate referrals accordingly.

The ideal candidate will have strong knowledge of healthcare delivery, HIPAA compliance, and experience with EMR systems.

Qualificações

HS Diploma or equivalent required; healthcare coursework preferred.
2 years experience in referral management or care coordination.
Medical assistant experience helpful but not required.
Experience with EMR systems, preferably eClinicalWorks or similar.

Responsabilidades

Partner with care team to understand member care needs and gaps in treatment.
Coordinate referrals to external providers and services.
Document referrals to ensure records are accurate and current.
Maintain relationships with members and families to facilitate care coordination.
Monitor progress on care plans and follow up on goals.
Adhere to HIPAA and protect PHI.
Promote clear communication among care team and clinicians.
Assist members in overcoming barriers to care (interpreter services, transportation, etc.).
Act as system navigator and point of contact for members and families.
Perform additional duties as assigned.
