Job summary

ArchWell Health is seeking a Care Navigator to welcome members at our Phoenix centers and ensure a smooth, person-centered experience from check-in to departure. You will update records, coordinate referrals, and assist with paperwork for a multidisciplinary care team, while providing exceptional service to diverse member families.

Fluency in Spanish is preferred to support our diverse communities; prior EMR experience and strong communication skills are highly valued in this role.

Qualifications

• Associates degree preferred, or equivalent experience.
• Minimum one year of work experience in a clinical or similar setting.
• Experience with EMR systems is a plus.

Responsibilities

• Welcome members upon arrival and guide them through check-in.
• Update member information in the electronic medical records system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload documentation to EMR.
• Field questions from members and their families; manage incoming calls.
• Assist with center events as needed.
