Job summary

This role emphasizes accountability, health coaching, and cost-effective care within a value-based system. As a leader in the center, you will recruit patients, collaborate with APPs, and participate in center management.

Qualifications

• MD/DO in Internal Medicine, Family Medicine, or Geriatrics.
• State medical license in the applicable state.
• Board certification preferred; board eligibility required.
• Current DEA number for controlled substances.
• BLS certification within 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take accountability for patient care and outcomes.
• Engage hospitalists when patients are in hospital.
• Manage assessment, diagnosis, treatment, and care coordination.
