Job summary

ChenMed, a physician-led primary care organization, seeks Primary Care Physicians to join our mission-driven team. The PCP will manage a patient panel, lead a care team, and coordinate care across settings in a value-based model.

We emphasize accountability, coaching for health, and actions to simplify care for patients. Travel within local markets may be required to support centers and care teams.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Active state medical license or ability to obtain one in the applicable state.

Responsibilities

• Provide care for patients with acute and chronic illnesses in older adults.
• Take accountability for patient care and outcomes, consult with specialists as needed.
• Engage with hospitalists when patients are in hospital.
• Manage assessment, diagnosis, treatment, health education, and care coordination.
• Lead care team including medical assistant, care facilitator, and care coordinator.
