Creative Solutions In HealthCare in McKinney, TX is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will evaluate patients, develop personalized plans, and administer PT treatments to enhance mobility and quality of life.

Qualifications

• Active Texas Physical Therapist license in good standing.
• Experience in SNF or long-term care settings preferred.
• Solid knowledge of current physical therapy practices.
• Familiarity with Net Health documentation system preferred.
• Strong communication and collaborative clinical decision making.

Responsibilities

• Consult with patients to assess their condition and functional limits.
• Evaluate and interpret assessments and test results.
• Develop individualized treatment plans using various techniques.
• Create personalized rehabilitation programs aligned with goals.
• Administer prescribed PT treatments to relieve pain and restore function.
• Ensure treatments follow the care plan and state/federal regulations.
• Monitor responses and adapt approaches with the care team.
• Provide education to patients, families, and caregivers.
• Document thoroughly and timely per company policies.
