Job summary

Creative Solutions in Healthcare, Inc. is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house team.

You will assess patients, develop individualized treatment plans, and implement rehabilitation programs to improve mobility and quality of life in residents. You will collaborate with the interdisciplinary team, document thoroughly, and ensure care aligns with the plan while guiding families and caregivers.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Knowledge of PT treatment practices.
• Experience with Net Health documentation preferred.
• Ability to manage diverse patient needs.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to assess physical condition and limitations.
• Evaluate assessments and test results.
• Develop individualized treatment plans with various techniques.
• Create personalized rehab programs aligned with patient goals.
• Administer prescribed PT treatments to relieve pain and restore function.
• Ensure treatments align with the care plan.
• Monitor patient responses and adjust approaches with the team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation per policies and regulations.
• Attend required meetings as designated by leadership.
