Qualifications

• Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.

Responsibilities

• Perform comprehensive in-person assessments to develop personalized care plans.
• Coordinate discharge for acute, short-term, and long-term placement.
• No hospital rounds required.

Skills

Successful completion of residency program

Board certification preferred

Active state physician license

State CDS certification and DEA registration

CPR and First Aid certification

Education
