Job summary

A Nurse Practitioner role at a PACE center opening in the Stockton, CA area offers primary care to frail elderly participants with complex conditions. You will work with an interdisciplinary team to assess, diagnose, and manage care, developing individualized plans and coordinating across center visits, home care, and referrals.

Responsibilities include testing, medication management, documentation in the EMR, and participating in IDT meetings and CMS compliance.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Active, unrestricted California NP license, or ability to obtain prior to start.
• National certification in Family, Adult-Gerontology, or Geriatric practice preferred.
• Current DEA registration, or ability to obtain.
• Experience with geriatric or medically complex populations preferred.

Responsibilities

• Provide primary care services to a panel of PACE participants with complex chronic conditions.
• Conduct comprehensive health assessments and develop treatment plans.
• Collaborate with IDT and coordinate patient care across settings.
• Order and interpret tests, prescribe and manage medications within scope.
• Maintain EMR documentation and participate in QA and CMS compliance.
• Participate in on-call rotation as needed.
