Job summary

Gary and Mary West PACE is seeking a Nurse Practitioner to provide primary and acute care for seniors in a multidisciplinary PACE program. The role supports geriatric patients in the center and in participant homes, with a focus on collaborative care planning and evidence-based management.

As part of the IDT, you will conduct comprehensive assessments, coordinate with physicians, and participate in rotating night and weekend calls, ensuring high-quality care while complying with PACE

Qualifications

• Graduate of an accredited school of nursing.
• RN State License.
• Certification as a Nurse Practitioner with prescriptive authority.
• DEA registration.
• Current BLS and CPR training.
• Valid driver's license and auto insurance.

Responsibilities

• Provide primary care and acute illness evaluation in center and at participants' homes.
• Conduct in-person comprehensive assessments and develop care plans with the IDT.
• Coordinate discharge planning from acute or long-term care settings.
• Refer participants to medical specialists as indicated.
• Manage care in the nursing home with regular visits per GMWP standards.
• Participate in rotating night and weekend calls.
• Review outcomes and utilization data to improve practice and reduce hospitalizations.
• Maintain privacy and follow company policies.
