Job summary

The Career Land Center, LLC in the Stockton, CA area is seeking a Nurse Practitioner to join its interdisciplinary care team at a newly opening PACE center. You will provide primary care to frail, elderly participants in collaboration with the supervising physician.

Responsibilities include comprehensive health assessments, care planning, conducting IDT meetings, coordinating care across settings, prescribing within scope, documenting in the EMR, and supporting quality improvement within CMS and

Qualifications

• Master's or Doctoral degree in Nursing from an accredited NP program.
• Active, unrestricted California NP license, or ability to obtain prior to start.
• National certification (ANCC or AANP) in Family, Adult-Gerontology, or Geriatric practice preferred.
• Current DEA registration, or ability to obtain.
• Experience with geriatric or medically complex populations preferred.
• Prior experience in a PACE program or similar interdisciplinary care model preferred.

Responsibilities

• Provide primary care services to a panel of PACE participants in collaboration with supervising physician.
• Conduct comprehensive health assessments and develop individualized treatment plans.
• Serve as an active member of the interdisciplinary care team and participate in IDT meetings.
• Coordinate participant care across settings including center visits, home care, and referrals.
• Order, interpret diagnostics, prescribe and manage medications within scope of practice.
• Maintain accurate documentation in the electronic medical record.
• Support quality improvement initiatives and CMS/PACE compliance.
• Participate in an on-call rotation as needed.
