Job summary

Gary and Mary West PACE seeks a Physician Assistant to provide primary and specialty care to frail elderly participants in a multidisciplinary setting. You will perform assessments, coordinate care plans with the IDT, and support home and center-based visits, ensuring high-quality geriatric care.

You’ll work under Medical Director supervision, participate in rotating on-call duties, and maintain licensure while contributing to policy-driven improvements in a mission-driven, team-oriented culture.

Qualifications

• Graduate of an accredited Physician Assistant program.
• Physician Assistant State License.
• Valid DEA registration.
• Certification as a Physician Assistant (PA) with prescriptive authority.
• Current Basic Life Support (BLS) and CPR training.
• Current driver’s license and clean driving record.

Responsibilities

• Provide primary care and acute illness evaluation and treatment for participants in the center and in participants' homes.
• Perform in-person comprehensive assessments with the IDT to develop comprehensive care plans.
• Collaborate with physicians and the IDT on discharge planning from acute or long-term care settings.
• Refer participants to medical specialists as indicated.
• Manage care of participants in the nursing home and conduct regular visits.
• Participate in rotating night and weekend calls.
• Review outcomes and utilization data to improve care and reduce hospitalizations.
• Maintain privacy and confidentiality of all information; follow policies and procedures.
