Job summary
West PACE in California seeks a Nurse Practitioner to provide primary and acute care for PACE participants. You will operate within an interdisciplinary team, coordinating care with PACE physicians and IDT in the center and participants' homes.
The role requires NP with prescriptive authority, DEA, and BLS/CPR. Strong geriatrics knowledge, communication, and care coordination are essential; weekend/night coverage may be required.
Qualifications
- Graduate of an accredited school of nursing.
- RN State License.
- Certification as a Nurse Practitioner with prescriptive authority.
- Valid DEA.
- Current BLS and CPR training.
- Current driver's license and proof of auto insurance.
Responsibilities
- Provide primary care and acute illness evaluation and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians in the center and participant homes.
- Perform in-person comprehensive assessments and coordinate with IDT to develop care plans.
- Discharge planning in collaboration with PACE Physician and IDT.
- Refer participants to medical specialists as indicated.
- Manage care in nursing homes and make regular visits as per standards.
- Be an active IDT member communicating changes and coordinating 24-hour care.
