Greater Good Health is seeking a Nurse Practitioner in Maryland to join our team supporting Medicare patients across in-clinic and in-home visits with some telehealth as needed. The role emphasizes thorough patient engagement, education, and coordinated care within an integrated team.

Applicants must hold a Maryland NP license, ANCC/AANP certification, and DEA prescriptive authority, with 2+ years of clinical NP experience and CPR/BLS. A flexible schedule is available within a hybrid model.

Qualifications

• Maryland NP license required.
• NP certification from ANCC or AANP.
• 2+ years NP clinical experience in primary care, family/gerontology or palliative care.
• DEA prescriptive authority post-hire and CPR/BLS current.
• Knowledge of HCCs and chronic care management.

Responsibilities

• Support highly complex patients navigating the healthcare system.
• Provide patient education and care plans with an integrated team.
• Coordinate care with dialysis/ nephrology teams.
• Conduct health assessments and accurate clinical documentation.
• Maintain HIPAA confidentiality and participate in STAR gap closure.

Education

Maryland Nurse Practitioner license

ANCC/AANP NP certification

2+ years clinical NP experience

Active CPR/BLS
