Qualifications

• Nurse Practitioner certification from ANCC / AANP.
• 2+ years of clinical NP experience in primary/family/internal gerontology or palliative care.
• Working knowledge of chronic care management and medications.
• Current DEA licensure / prescriptive authority after hire.
• Active CPR/BLS certification.
• Experience with HCCs.

Responsibilities

• Support complex patients navigating the healthcare system with one-on-one care.
• Provide patient education, care plans, and clinical guidance to the care team.
• Collaborate with Integrated Care to serve dialysis patients.
• Coordinate care across patients, families, physicians, and providers.
• Administer tests and screenings.
• Perform health assessments with accurate documentation.
• Maintain HIPAA confidentiality.
• Contribute to STAR gap closure.
