Carease Health is seeking an experienced Nurse Practitioner (NP) with a geriatrics or internal medicine background to deliver preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Success in this role depends on building strong relationships with facilities, maintaining a patient-centered approach, and being a collaborative team player.

Qualifications

• Active NP or PA license (in the state of practice).
• 1+ year of experience in geriatric care or internal medicine.
• Experience with documentation review and care coordination.
• Excellent communication, collaboration and teamwork skills.

Responsibilities

• Deliver high-quality preventative care to residents in skilled nursing facilities.
