Job summary

Greater Good Health is seeking a Nurse Practitioner to support high-need senior patients in a hybrid role based in Annapolis, MD. You will provide patient education, develop care plans, and collaborate with an integrated care team focused on dialysis patients and value-based outcomes.

The ideal candidate holds an active Maryland NP license with ANCC/AANP certification and 2+ years of relevant experience, with DEA prescriptive authority and CPR/BLS.

Qualifications

• Current Maryland Nurse Practitioner license.
• NP certification from ANCC or AANP.
• 2+ years of NP experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• DEA licensure / Prescriptive Authority post-hire.
• Active CPR/BLS Certification.
• Must have knowledge/history working with HCCs.
• Current Maryland Driver’s License.

Responsibilities

• Support highly complex patients, guiding them through a complex healthcare system.
• Provide patient education, care plans, and clinical guidance to an integrated care team.
• Collaborate with Integrated Care team to serve dialysis patients.
• Coordinate care with physicians, family, and other providers.
• Administer tests and screenings, document findings accurately.
• Conduct comprehensive and health risk assessments with proper coding and documentation.
