Job summary

Curana Health in North Carolina is seeking a qualified Advanced Practice Provider to deliver holistic, compassionate Medicare Advantage care within our network of senior living communities and facilities. You will work with physicians, Care Coordinators, RNs, and other health professionals to assess risks, manage plans of care, and promote wellness while navigating regulatory requirements.

Qualifications

• Valid NP or PA license in the operating state.
• Master's degree in nursing or PA studies required.

Responsibilities

• Performs health risk assessments on plan members.
• Collaborates with physicians, Care Coordinators, staff and family members.
• Identify quality improvement activities and plan care.
• Review medical histories and interpret data to guide treatment.
• Prescribe or perform treatments including medications and counseling.
• Ensure compliance with OSHA and state nursing boards.
• Refer members to PCP and disease management programs.
• Maintain HIPAA privacy and protection of PHI.
• Provide feedback to improve operations.
