Job summary

Greater Good Health is seeking a Nurse Practitioner to deliver high-quality, holistic care to senior Medicare patients across in-clinic and in-home settings. You will work with nephrology partners and an integrated care team to develop care plans, educate patients, and address complex health needs.

The role emphasizes patient-centered care, strict HIPAA compliance, and flexibility to cover regions as needed. Eligible candidates must be licensed in NE and able to travel within service areas.

Qualifications

• Nurse Practitioner certification from ANCC / AANP.
• 2+ years clinical NP experience in Primary Care/Family/Internal/Gerontology/Palliative Care; home care a plus.
• Knowledge of chronic care management and medications.
• Current DEA licensure / prescriptive authority post-hire.
• Active CPR/BLS certification.
• Knowledge/history with HCCs.

Responsibilities

• Support highly complex patients, helping navigations through complex care.
• Provide patient education, care plans, and clinical guidance to the care team.
• Collaborate with Integrated Care for dialysis patient population.
• Work autonomously and with the health care team to coordinate high-quality, cost-effective care.
• Coordinate care between patient, family, physicians, and other providers.
• Administer tests and screenings.
• Perform Comprehensive Health Assessments and Health Risk Assessments with precise coding/documentation.
• Maintain HIPAA confidentiality.
• STAR gap closure.
