Job summary

Optum is seeking a Home Visit Nurse Practitioner to provide primary care in patients' homes across several counties. You will collaborate with physicians and care teams to assess, plan, and manage patient care during in-home visits and ensure patient-centered outcomes.

The role requires active NP licensure, certification (ANCC/AANP), and ability to travel extensively within the local market, with a 75% travel expectation. A sign-on bonus is offered for external candidates.

Qualifications

• Active, un-encumbered nursing license in the state of assignment.
• Nurse Practitioner certification through ANCC or AANP.
• DEA license or ability to obtain.
• Basic Cardiac Life Support (BCLS) certification.
• Access to reliable transportation to travel to patient sites.

Responsibilities

• Obtain and review medical history, perform assessments, diagnose conditions, and plan care.
• Identify gaps in care and refer appropriately based on findings.
• Develop interventions to help members achieve care goals.
• Monitor progress and reassess care planning with the team.
• Communicate effectively with member, family, and care team; build trust.
