Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care for seniors in long-term care settings. You will manage a panel of enrolled members, perform assessments, and develop individualized care plans with a collaborative clinical leadership team.

You will engage with families, coordinate care with the interdisciplinary team, and help minimize hospitalizations while delivering patient-centered services in a supportive nursing home environment.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience working in nursing home environment is strongly desired.
• State licensure and national certification required with active credentials.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Facilitates health plan operations, prior authorizations, and quality initiatives.
• Serves as trusted advisor to members, families, and staff.
• Communicates care plans with PCPs, families, and interdisciplinary teams.
• Maintains appropriate caseloads and aligns with organizational goals.
• Promotes Longevity Health in clinical and nursing facility settings.
• Functions autonomously and develops meaningful relationships in the nursing home environment.
• Ensures high-quality, member-centric care and performs related duties as assigned.
