Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of nursing home members in Columbus, GA. You will conduct comprehensive assessments, develop individualized care plans, and collaborate with a multidisciplinary team to deliver high-quality care.

You will work autonomously within a clinical leadership framework, ensuring alignment with members’ goals and needs while maintaining frequent communication with families and care teams in a long-term care setting.

Qualifications

6+ months experience as a Nurse Practitioner or Physician Assistant.
MS in Nursing as a NP from a nationally accredited program OR Master’s degree from a nationally accredited program.
Experience working in the nursing home environment is strongly desired.
DEA License required; active NPI and Medicare provider numbers are required.

Responsibilities

Provides collaborative primary care and services to a panel of members.
Delivers ISNP Model of Care activities and develops advanced care plans to avoid unnecessary hospitalizations.
Facilitates health plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS/STARs quality gap closure; promotes membership growth.
Serves as a trusted advisor to members, families, and nursing home staff and colleagues.
