Qualifications

• Experience in healthcare, social services, customer service, admissions, or case management preferred.
• Strong organizational and time-management skills.
• Ability to coordinate multiple appointments and schedules simultaneously.

• Computer proficiency and experience with documentation systems preferred.
• Professional, dependable, and compassionate attitude.
• Valid driver's license preferred.

Responsibilities

• Coordinate and schedule resident transportation for medical appointments, therapies, and outside services.
• Communicate with families, healthcare providers, hospitals, and transportation vendors.
• Assist the Social Services department with resident needs, admissions, discharges, and care planning support.
• Help residents access community resources and services as appropriate.
• Maintain accurate records, schedules, and documentation.
• Provide excellent customer service to residents, families, and visitors.
• Serve as a positive resource and advocate for residents throughout their stay.
• Ensure transportation arrangements are completed efficiently and on time.
