Job summary

Marin Post Acute in San Rafael, CA seeks a Social Services Director to lead our resident-centered social services team. You will guide discharge planning, coordinate admissions and transitions, advocate for resident rights, and work with physicians, nursing, therapy, and community agencies to optimize outcomes.

This role requires a Bachelor's in Social Work or related field, SNF/healthcare experience preferred, and prior leadership.

Qualifications

• Bachelor's Degree in Social Work, Human Services, Psychology or related field required.
• SNF/rehabilitation/healthcare experience preferred.
• Previous leadership or supervisory experience strongly preferred.
• Strong knowledge of discharge planning, care coordination, and resident advocacy.

Responsibilities

• Direct and oversee all aspects of the Social Services Department.
• Develop, coordinate, and implement resident-centered social service programs.
• Assist residents and families with admissions, care planning, transitions, and discharge planning.
• Advocate for resident rights and ensure regulatory compliance.
• Collaborate with physicians, nursing, therapy teams, and community agencies to support outcomes.
• Provide counseling, emotional support, and crisis intervention as needed.
• Maintain documentation and participate in interdisciplinary care team meetings.
