Job summary

Your Health Organization is seeking a Senior Solutions Advisor to assist patients in the [LOCATION] area. This role travels to patients' homes and facilities daily as part of a full-time schedule from 8:00 AM to 5:00 PM, Monday through Friday, supporting care transitions and coordinating with local resources.

• Bachelor's degree in a related field.
• Experience in case management, senior services, or healthcare navigation preferred.
• Strong interpersonal and written skills.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support challenges.
• Provide guidance on living arrangements, safety, and available Your Health services.
• Serve as the regional point of contact for onboarding new patients.
• Coordinate with local care resources to ensure access and continuity of care.
• Educate community professionals and family caregivers through outreach and training.
