Job summary

Your Health is seeking a Senior Solutions Advisor to support patients across the Myrtle Beach area, delivering person-centered guidance through aging, chronic illness, and care transitions. This field-based role requires travel to homes and facilities and follows a standard 8-hour workday, Monday–Friday.

You will assess long-term care needs, connect patients to local resources and services, and educate families and providers about Your Health offerings.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Healthcare Administration, or related field.
• Prior experience in case management, senior services, or healthcare navigation preferred.
• Strong interpersonal and communication skills with the ability to build trust with families and providers.
• Self-starter with proven time management, coordination, and follow-through skills.
• Valid driver’s license and reliable transportation required.
• Ability to read and communicate effectively.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to lead and foster teamwork.
• Willingness to travel between offices and facilities.

Responsibilities

• Meet with patients and families to assess long-term care needs.
• Provide consultative guidance on living arrangements and safety.
• Serve as the regional point of contact for onboarding new patients.
• Maintain knowledge of local senior care resources.
• Coordinate access and communication between patients and resources.
• Educate community professionals and families through outreach.
• Timely communication with new patients in Care Group.
• Support care transitions with transitional care providers.
• Assist families in accessing care management and hospice services.
• Document activities for billing purposes (CCM, ACP, etc.).
• Participate in community events and health fairs.
• Collaborate to support market expansion through referrals.
