Job summary

Your Health Organization, a leading physician group serving South Carolina and Georgia, seeks a Senior Solutions Advisor to assist patients in the designated area. This is a full-time role traveling to patients' homes and facilities, with 8:00 AM–5:00 PM hours.

The SSA connects patients and families with healthcare and community services, supports care transitions, and educates professionals to promote coordinated, compassionate care while growing our services through education and relationship

Qualifications

• Bachelor’s degree required or preferred in a related field.
• Experience in case management, senior services, or healthcare navigation is strongly preferred.
• Ability to build trust with families and providers, and to communicate effectively.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support challenges.
• Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
• Serve as regional point of contact for onboarding new patients.
• Maintain knowledge of local senior care resources and services.
• Coordinate access and communication between patients, families, and care resources.
• Educate community professionals and family caregivers through outreach and training.
• Communicate timely with new patients within the assigned Care Group or Team.
• Support care transitions with Transitional Care Providers and interdisciplinary teams.
• Assist families in accessing care management and hospice services.
• Document activities for billing purposes (CCM, ACP, etc.).
• Participate in community events to promote SSA services.
