Job summary

Your Health Organization in the United States seeks a Senior Solutions Advisor to assist patients across the designated service area. The SSA will navigate aging and chronic illness needs, connecting families to healthcare, community services, and caregiving options.

The role requires traveling to patients’ homes and facilities daily, working full-time from 8:00 AM to 5:00 PM, Monday through Friday, and collaborating with care groups to promote patient-centered solutions.

• Ability to manage multiple responsibilities with sound judgment.
• Self-starter with good time management and coordination.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support challenges.
• Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
• Serve as regional onboarding point for new patients.
• Maintain knowledge of local senior care resources and coordinate access.
• Educate community professionals, families, and providers through outreach and training.
• Communicate timely with new patients and support care transitions with care teams.
• Assist families in accessing care management and hospice services.
