Job summary

Your Health Organization is seeking a Senior Solutions Advisor to support patients across South Carolina and surrounding areas. The role travels to patients in homes and facilities, working 8 hours daily, Monday through Friday, to connect patients with appropriate healthcare and community resources.

The SSA will partner with care teams to educate families, coordinate services, and assist in care transitions, while building relationships to grow our outreach in the region.

Qualifications

• Bachelor’s degree or equivalent in related field.
• Experience in case management, senior services, or healthcare navigation preferred.
• Strong interpersonal and communication skills with families and providers.
• Self-starter with time management, coordination, and follow-through skills.
• Valid driver’s license and reliable transportation.
• Ability to read and communicate effectively; strong written and verbal skills.
• Basic computer knowledge.
• Ability to lead and foster a cooperative work environment.
• Flexible hours and travel between offices/facilities.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support.
• Provide guidance on living arrangements, safety, and available Your Health services.
• Serve as regional onboarding point for new patients.
• Know local senior care resources and services.
• Liaise between patients, families and resources for access and coordination.
• Educate community professionals and caregivers through outreach.
• Maintain timely communication with new patients in assigned Care Group.
• Support care transitions with Transitional Care Providers.
• Assist families in accessing care management and hospice services.
• Document activities for billing and Medicare services.
• Participate in community events and health fairs.
• Collaborate with Director of Development for market expansion.
