Job summary

AltaMed Health Services is seeking a coordinator to assist the PACE IDT in delivering essential services and ensuring participant satisfaction. You will handle inquiries, service requests, eligibility issues, complaints, and appeals, while documenting incidents and processing disenrollments.

Strong candidates will have a high school diploma with 2+ years of customer service, knowledge of Medi-Cal/Medicare, bilingual skills, and a valid BLS certification. Healthcare experience is preferred.

Qualifications

• High School diploma with at least 2 years of customer service experience.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits; PACE knowledge preferred.
• Healthcare setting experience serving frail or elderly preferred.
• Bilingual Spanish/English, Chinese/English, or another second language preferred.
• Valid BLS certification following AHA or Red Cross guidelines.

Responsibilities

• Assist the PACE IDT in coordinating essential PACE services.
• Assist participants with inquiries, service requests, eligibility issues, complaints, and appeals.
• Document and process participant incident reports and involuntary disenrollments.
• Facilitate on-site satisfaction surveys and participant experience activities.
• Link participants to community resources and coordinate referrals with interdisciplinary teams.
