Job summary

AltaMed Health Services is seeking a PACE IDT Coordinator to assist in coordinating essential PACE services and ensure high participant satisfaction through strong customer service. You will handle inquiries, eligibility issues, complaints, and appeals, while documenting incidents and processing disenrollments.

Collaboration with interdisciplinary teams is essential to meet program goals. Knowledge of Medi-Cal/Medicare and a bilingual capacity are strongly preferred; a high school diploma is

Qualifications

• Minimum 2 years of customer service experience required.
• Bachelor's degree preferred.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits required.
• PACE program knowledge preferred.
• Experience in PACE operations managed care or healthcare settings serving frail or elderly preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Assist the PACE IDT in coordinating essential PACE services.
• Support participant inquiries, service requests, eligibility issues, complaints and appeals.
• Document and process participant incident reports and involuntary disenrollments.
• Facilitate on-site satisfaction surveys and participant experience activities.
• Assist with linkage to community resources and referrals as needed.
• Collaborate with interdisciplinary teams to meet program and organizational goals.
