One Medical is hiring a Flex Health Navigator to support senior patients (65+) on Medicare, focusing on care coordination and navigation across virtual and in-person interactions. You will bridge patients with providers and care teams to ensure seamless care and improved health outcomes in a dynamic healthcare environment.

Qualifications

• At least 1 year of experience in a high touch patient-facing healthcare role.
• Strong EMR system proficiency and comfort using healthcare technology platforms.
• Proficient in Google Workspace (Sheets, Docs, Slides) and adept at navigating multiple software systems.
• Excellent written and verbal communication skills with demonstrated ability to collaborate effectively on diverse, multi-disciplinary teams.
• Self-motivated with proven ability to work independently, adapt to change, and proactively address challenges under pressure.

Responsibilities

• Coordinate comprehensive care for senior patients including referrals, appointments, and follow-up with high-risk or disengaged patients.
• Conduct pre-visit screenings and post-visit check-ins to identify care priorities and ensure continuity across care teams.
• Support preventive care by monitoring patient panels, cognitive screenings, and addressing care gaps through outreach.
• Provide patient education on chronic disease management using motivational interviewing and action-planning techniques.
• Contribute to daily clinic operations, including phone support, front desk assistance, and maintaining clinical spaces.
