Job summary

MissionHires is looking for a Senior Care Advocate (LCSW/RN) to support Medicare members by providing care coordination, overcoming barriers to access, and connecting them with essential community resources. The ideal candidate will have strong community health work and supervisory experience.

Most of the role is virtual, with occasional community visits. Responsibilities include managing member communications and developing care plans that address social needs.

Qualifications

• Active, unrestricted licensure in good standing as a LCSW or RN in Florida.
• Minimum 4 years of experience in case management, care navigation, and patient advocacy.
• Extensive knowledge of community-based resources and social service programs.

Responsibilities

• Build strong, trusting relationships with Medicare members.
• Manage day-to-day workload communicating with members through various channels.
• Connect members to community-based social services.
• Assess and prioritize members’ needs and support them in navigating healthcare systems.
• Develop care plans addressing social determinants of health.
