As a One Medical Flex Health Navigator supporting our senior patients (age 65+) on Medicare, you are focused on delivering exceptional care coordination and team assistance. This role is primarily serving patients with complex care needs who benefit from coordination, advocacy, and navigation through both virtual and in‑person interactions. As a Health Navigator, you will serve as a crucial link between patients, our providers, and the broader healthcare system, ensuring seamless care delivery and improved health outcomes. Our Flex Health Navigators lend support to multiple offices in an MSA.
You are passionate about improving healthcare outcomes for seniors, and excel at building trust and rapport with patients. You demonstrate strong problem‑solving abilities, and adapt well to changing priorities and environments. You have a commitment to continuous learning and improvement to facilitate overall better healthcare outcomes for our patients.

This is a full‑time role (40 hours/week Monday – Friday from 8am‑5pm) based in offices in Los Angeles, CA. Flex Health Navigators lend support to multiple One Medical locations in an MSA and location and schedule are subject to change.
Base pay: $22.75 per hour based on a full‑time schedule.
