Job summary

CHRISTUS Health in Irving, TX seeks an RN Care Manager II to lead patient care planning from admission through discharge. You will coordinate the care continuum with physicians and the team, monitor progress, and remove barriers to timely discharge while ensuring effective resource use.

The role requires a BSN, strong communication, and analytical skills; Master’s preferred and experience in care management and utilization management is a plus.

Qualifications

• BSN from an accredited nursing program or demonstrated success in CHRISTUS RN Care Manager I.
• Master’s degree in nursing preferred.
• Experience managing multiple projects simultaneously is a plus.
• Excellent verbal and written communication.
• Analytical thinking and clinical competency.

Responsibilities

• Meets expectations of CHRISTUS leaders and coordinates patient care across the continuum.
• Implements and monitors the plan of care for effectiveness and appropriateness.
• Identifies and escalates barriers impeding diagnostic or treatment progress.
• Proactively resolves delays and obstacles to discharge.
• Collaborates with medical, nursing, and ancillary staff to eliminate barriers to care.
• Ensures plan communication to patient/family and care team for continuity.
