Job summary

CHRISTUS Health is seeking a Care Manager II to coordinate patient care from admission through discharge, collaborating with physicians, nursing and ancillary staff to optimize throughput and safe transitions. You will assess patient needs, develop individualized care plans, and communicate effectively with patients, families, and care teams.

Qualifications

• RN licensure or LMSW/LBSW credentials as required by CHRISTUS policy.
• Experience in discharge planning and utilization management highly desired.
• Strong communication, collaboration, and patient advocacy skills.

Responsibilities

• Coordinate care progression and discharge planning with the multidisciplinary team.
• Advocate for patients regarding treatment decisions and post-acute care options.
• Identify and remove barriers to discharge to improve throughput and patient safety.
• Monitor length of stay and collaborate with physicians and payors as needed.
• Document care plans and ensure regulatory compliance throughout the patient journey.
