Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) to collaborate with an RN in delivering high-quality nursing care and improving residents' quality of life. The LPN will be a key member of the clinical team, supporting culture change and mentoring others on the unit.

The role emphasizes patient dignity, autonomy, and participation in daily activities, with an ongoing commitment to safe, efficient, and compassionate care aligned with HSL policies.

Competenze

• Graduate of approved school of nursing; current Massachusetts LPN license.
• Current BLS certification required.
• Collaborates with RN and multidisciplinary team to meet patient needs.
• Commitment to patient-centered care and quality outcomes.
• Experience in geriatric/long-term care preferred.

Mansioni

• Ensure nursing care delivered follows safe standards and practices.
• Collaborate with MD, NP, nurses, social worker, therapists, and dietician to meet patient needs.
• Apply evidence-based interventions to optimize outcomes.
• Work in a primary nursing model with RN oversight for care planning.
• Document care in EMR and participate in quality improvement activities.
• Mentor and coach patient care associates; may act as charge nurse with RN oversight.
