Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) to join the clinical support team on-site in Massachusetts. You will perform nursing care under RN supervision, participate in care planning, and document using the EMR while supporting patient autonomy and dignity.

You will collaborate with multidisciplinary team members, coach and supervise care associates, and contribute to performance improvement initiatives within a culture of safety and quality care.

Qualifications

• Graduate of approved school of nursing.
• Current Massachusetts LPN license in good standing.

• Excellent English communication, both verbal and written.
• Able to manage multiple tasks and changing priorities.

Responsibilities

• Provide nursing care under RN supervision.
• Collaborate with MD, RN, social worker, therapists, and dietitian.
• Document care using the EMR; ensure accurate records.
• Participate in care planning activities and support patient autonomy.
• Delegate and supervise tasks to the nursing assistant; coach PCAs.
• Contribute to performance improvement and shared governance.
