Job summary

Hebrew SeniorLife in Massachusetts is seeking a Licensed Practical Nurse (LPN) to work collaboratively with a Registered Nurse to provide compassionate, quality nursing care across units. The LPN will participate in the nursing process, advocate for patient dignity, and support culture change initiatives, assisting with care plans under RN oversight.

Qualifications

• Graduate of an approved school of nursing is required.
• Current Massachusetts license as an LPN in good standing is required.
• Excellent English language skills, verbal and written.
• Ability to adapt to changing priorities and exercise good judgment.
• Strong organizational and analytical skills.

Responsibilities

• Ensure care delivered is within the scope and established standards of safe professional nursing practice with core care competencies.
• Collaborates with MDT members (MD, NP, nurses, social worker, therapists, dietitian) to meet patient needs and monitor outcomes.
• Adheres to best practice recommendations and implements evidence-based interventions for optimum outcomes.
• Practices in a primary nursing model with RN oversight of assessment and care planning.
• Documents care in the EMR and initiates necessary reports (DPH, incident, concern resolution).
• Delegates and supervises care delivered by the nursing assistant; maintains accountability for own learning and development.
