Job summary

Hebrew SeniorLife is seeking a Licensed Practical Nurse (LPN) to join our clinical team on-site in Massachusetts. You will work with a Registered Nurse using the nursing process to provide safe, high-quality care and support patient independence and dignity.

You will collaborate with a multidisciplinary team, mentor patient care associates, and participate in governance and performance improvement initiatives while upholding HIPAA and care standards.

Qualifications

• Graduate of an approved school of nursing is required.
• Current Massachusetts license as an LPN in good standing.
• Previous nursing experience preferred.
• Understand the philosophy and support the model of care at HSL.
• Must be professional, collaborative and results oriented.

Responsibilities

• Ensure care delivered is within safe professional nursing practice.
• Collaborate with multidisciplinary team to meet patient needs.
• Adhere to evidence-based interventions for optimum outcomes.
• Work in a primary nursing model with RN oversight.
• Document care in EMR and generate necessary reports.
• Coach and mentor patient care associates.
