Hebrew SeniorLife in Massachusetts seeks a Licensed Practical Nurse (LPN) to collaborate with a Registered Nurse to provide high-quality, patient-centered care. The LPN will participate in assessment, planning, implementation, and evaluation of nursing care, while supporting culture-change initiatives and mentoring other staff.

This role requires Massachusetts LPN licensure, excellent communication, and the ability to work with a multidisciplinary team in a dynamic clinical environment.

Qualifications

  • Graduate of an approved nursing program and eligible for Massachusetts LPN license.
  • Professional, proactive, collaborative and results-oriented.
  • Excellent English communication, both spoken and written.

Responsibilities

  • Provide nursing care within the scope and established standards of safe practice.
  • Collaborate with the multidisciplinary team (MD, NP, nurses, social worker, therapists, dietician) to meet patient needs.
  • Document care delivered in the electronic medical record and report safety concerns.
  • Participate in care planning activities and support autonomy where possible.
  • Delegate and supervise nursing assistants; maintain accountability for own learning and development.
  • May assume charge nurse responsibilities with RN oversight and participate in rounds.
