Brooksby Village by Erickson Senior Living seeks a Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to provide clinical care to residents in our neighborhood in Peabody, MA. The role emphasizes a resident-first approach and collaborative care within an expanding senior living community.

You will assess residents, document in the EMR, update care plans, supervise care associates, and work with RN and providers to ensure high-quality care across shifts.

Qualifikationen

• Minimum 1 year of experience in assisted living, home health, or long-term care preferred.
• Licensed by State as a Licensed Practical Nurse (LPN).
• CPR certification required.
• Passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records preferred.

Aufgaben

• Develop relationships with residents and families, placing the resident first.
• Assess, observe, and document residents’ clinical condition; notify RN/provider as needed.
• Update care plans to reflect changes in care needs.
• Supervise and lead care associates.
• Administer medications timely and accurately.
• Participate in meetings related to resident outcomes and care.
• Respond to residents/families professionally and promptly.
• Contribute to clinical indicator reviews (falls, wounds, nutrition, antipsychotic use).
• Other duties as assigned by RN clinical leader.
