The role emphasizes building relationships, assessing residents, updating care plans, supervising care associates, administering medications, and participating in care meetings to support residents’ physical, spiritual, and emotional well-being.

Qualifications

• Licensed by State as LPN/LVN.
• CPR certification required.
• Minimum 1 year of experience in assisted living, home health, or long-term care preferred.
• Experience with electronic medical records is preferred.

Responsibilities

• Provide clinical care to residents per standards and orders.
• Assess residents and document findings in EMR.
• Update care plans as needed.
• Supervise and lead care associates.
• Administer medications timely and accurately.
• Participate in resident outcomes meetings.
