Job summary

Episcopal SeniorLife Communities is seeking an Affordable Housing Service Coordinator to work 15 hours per week in two New York locations: Rockwood Center, Henrietta and Upper Street South, Geneseo. The role supports older adults in maintaining affordable housing and coordinating services with residents and providers.

Responsibilities include coordinating care, attending meetings, documenting encounters, and ensuring compliance with HUD standards.

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• Associate’s Degree in Social Work or related field required.
• Two years of social service delivery experience required.
• Knowledge of local services for the elderly and people with disabilities.

Responsibilities

• Coordinate care with residents, supports, and providers.
• Attend and facilitate meetings with residents and supports.
• Document encounters with residents and providers.
• Assist residents in maintaining housing in the affordable housing community.
• Refer residents to resources with consent.
• Complete required documentation and meet program timelines.
