Job summary

A Place At Home Montclair is seeking a Care Manager, Marketer and Community Educator to serve Essex County and surrounding areas. The role centers on building referrer relationships, educating families, and coordinating home care plans for seniors.

You will visit hospitals and clinics, run dementia/ Parkinson’s care trainings, and manage CRM-driven lead follow-up to ensure timely care starts. Strong presentation and organizational skills are essential.

Qualifications

• LMSW/LBSW preferred; licensure encouraged.
• Occupational Therapist, Physical Therapist, LPN, RN considered.
• Exceptional presentation skills and highly organized.
• Experience with PowerPoint and Excel.
• Experience with the elderly preferred.
• Passion for the elderly a must.

Responsibilities

• Identify local influence centers (hospitals, nursing homes, doctors' offices) to determine lead sources.
• Build and maintain a database of potential referrers.
• Achieve weekly visit frequency with key targets and deliver messaging.
• Conduct training sessions for professionals and families on dementia and Parkinson’s care.
• Visit and meet with referral sources (hospitals, clinics, non-profits) to discuss services.
• Arrange presentations of Home Care services at staff meetings and brokerages.
• Attend trade shows, conferences, and networking events for Home Care services.
• Network to develop additional lead sources.
• Manage on-call coverage with internal staff.
• Perform client assessments; input care plans into CRM; coordinate care starts.
