Job summary

A Place At Home, LLC in Montclair, NJ is recruiting for a Care Manager / Marketer / Community Educator to serve Essex County and surrounding areas. This role focuses on building relationships with referrers and families, delivering dementia and Parkinson's care messaging, and educating the community about home health services.

The ideal candidate will develop a robust network of hospitals, doctors, and senior-care providers while coordinating with internal staff to ensure timely patient start

Qualifications

• LMSW/LBSW preferred with experience in healthcare marketing.
• Excellent presentation skills and highly organized are a must.
• Experience with PowerPoint and Excel.

Responsibilities

• Identify all local influence centers (hospitals, nursing homes, facilities) to determine lead sources.
• Build and maintain a database of potential referrers.
• Meet weekly with manager to discuss referrals and activity.
• Conduct trainings for professionals and families on dementia and Parkinson's care.
• Visit and coordinate meetings with referrers including hospitals and private organizations.
• Present Home Care services at staff meetings, trade shows, and networking events.
• Attend events to develop additional lead sources.
• Use CRM to track leads and follow up on referrals.
• Coordinate with office staff to ensure care starts promptly.
