Job summary

East Arkansas Area Agency On Aging Inc. is seeking a Care Coordinator to facilitate care coordination for seniors and disabled individuals. This on-site, full-time hourly role reports to the Care Coordinator Supervisor and requires substantial driving to community locations.

The successful candidate will conduct needs assessments, develop service plans, maintain case records, advocate for clients, and collaborate with local organizations to promote available services and resources.

Qualifications

• Licensed in Arkansas as LMSW/LCSW or RN/LPN, or Bachelor's in health/human services with 2 years case management experience.

Responsibilities

• Perform needs assessment and develop individualized service plans in face-to-face interviews.
• Maintain accurate case records, including home conditions and service outcomes.
• Maintain frequent contact with clients to ensure services follow the care plan.
• Advocate for elderly and disabled rights and needs.
• Establish relationships with community resources to promote service utilization.
