Job summary

• Two to three years of experience in a home care setting.
• Minimum one-year home care experience preferred.

Responsibilities

• Assess, evaluate, and arrange services for older adults on the Targeted Care Management (TCM) program.
• Coordinate closely with Registered Nurses to support participant care and Medicaid/Medicare awareness.
• Notify participant and contracted vendors of approved service plans.
• Complete required paperwork for billing and maintain program records.
• Reassess participant needs through 90- or 180-day face-to-face reassessments.
• Collaborate with supports coordinators to ensure smooth transitions between service programs.
• Advocate for older adults.
